On Wednesday, Iran provocatively tested missiles that it says are capable of reaching Israel and a number of U.S. military bases. In connection with these tests, Sepah News, the media arm of the Iranian Revolutionary Guards posted this photograph of four missiles firing on its website.
By Thursday the photo was on the front pages of The Los Angeles Times, The Financial Times and The Chicago Tribune, as well as the websites of BBC News, MSNBC, Yahoo! News and NYTimes.com. On Friday, the Associated Press distributed a mostly identical photo with only three missiles.
As you can see, one of the missiles appears not to have launched. To cover their embarrassment it appears the Iranians photoshopped in a pretend missile.
